Click the above link to download the current Eagle Scout application. To use this form, you must have Adobe Reader or Adobe Acrobat. If you have a newer copy of Reader, you will be able to view the form and fill in the blanks and then print it out. Reader does not, however, have the ability to save your filled out form. The more powerful program, Adobe Acrobat, does have the ability to save your changes. Please note, this form does use colored text, so a good quality color printer should be used.

Eagle Scout Palm Application

Eagle Scouts who are ready to get their Palms will need to fill out an application. This application lists the merit badges earned to qualify for the Palm award, has a space for the Scoutmaster to sign signifying that the scout has qualified for the award, and spaces for the Board of Review members to sign. Ultimately this application is turned in to council.

This copy of the application is in Adobe PDF format. If you want, you can use the newer copies of Acrobat Reader to fill it in before printing it. In order to fill it in and save a copy on your PC, you will need Adobe Acrobat (not the Reader version). Most scouts who use this form will simply download it, fill it in and then print it out.

General Permission Slip Forms

Create a new Permission Slip:

If you would like to create a new permission slip for an upcoming event, grab this "RTF" format permission slip. It can be edited in Microsoft Word, the Windows Wordpad or many other document handlers. Edit whatever you need, fill in some of the blanks, print it out on your end, email it to whoever needs it, etc. Once it is filled in, you can email it to Tom Smith and she will get it posted here! Local Tour Permit from Northwest Suburban Council

This is the general permit form that must be filed with the local council to authorize a boy scout event, make sure insurance applies, etc.
